Treatment of epilepsy in Neurosphera

There’s a medical hierarchy at Neurosphera as in any other clinic or hospital. Each function is assigned specific tasks related to diagnosis and epilepsy treatment. The resident doctor is a doctor who undergoes a specialization in neurology. Resident doctors at Neurosphera, however, may boast a research and scientific output. At Neurosphera, they conduct ZERO appointments, during which a thorough medical history with the patient is taken and an analysis of all of the patient’s existing results is made.

A clinical specialist at Neurosphera is an experienced neurologist or epileptologist who monitors patients. In our team we have doctors who specialise in conducting epilepsy therapy for children and adults, with many years of work and experience gained at home and abroad.

A medical partner is the highest stage of professional experience. The medical partner is an acknowledged specialist in their field who guides Neurosphera medically. He is responsible for the implementation of modern treatment and for increasing access to the most modern world treatment methods for all of our patients.

Neurosphera’s medical staff is a closely cooperating team sharing knowledge and experience, so that every patient receives the most appropriate, individualised therapy and an ongoing care.

At Neurosphera we undergo the following tests related with epilepsy diagnosis and therapy:

  • EEG tests in children and adults in in the states of wakefulness, falling asleep and being asleep
  • VEEG (video EEG) in a multi-hour, multi-channel mode.
  • QEEG – mathematical analysis of EEG recordings using mapping, dipoles and fractal analysis
  • Polysomnograph

There’s a possibility of running those tests at home upon request.

At Neurosphera we provide not only the highest level of diagnosis and treatment of epilepsy, but also consultations with acknowledged specialists in related fields. Doctors other than neurologists and epileptologists, who cooperate with Neurosphera are referred to as clinical consultants and appointments with them are called CONSULTATION appointments. A pediatric neurosurgeon and a psychiatrist cooperate with Neurosphera.

All doctors working at Neurosphera who are dealing with diagnosis and epilepsy therapy are neurologists or undergoing a specialization in this field. They constitute a team which cooperates closely. Our attending physicians are acknowledged clinical specialists with a track record of work and participation in research projects at home and abroad, as well as of many publications. Neurosphera’s medical structure is arranged in such a way that every patient receives fully professional personalised support, appropriate to the stage of the disease, prognosis and expectations. The attending physicians can direct the patient to a CONSULTATION appointment to a doctor of another specialization who cooperates with Neurosphera. Thanks to the modern technology used in epilepsy diagnosis and therapy, all of the Neurosphera doctors will diagnose in the same way and propose the same therapeutic solutions.

The first appointment at Neurosphera is the ZERO appointment. It is usually conducted by Neurosphera’s doctor resident. The appointment takes the form of an in-depth medical history and an analysis of all of the data and results. On this basis, the resident doctor together with the clinical specialists and the medical partner determine the scale of the medical issue and make a preliminary diagnosis, about which the patient learns during the FIRST appointment. The patient’s attending physician is also selected on this basis. From now on, this doctor will be responsible for the first contact with the patient.

The attending physician is a specialist who will accompany the patient throughout the whole time of therapy at our centre. You can choose the specialist during your ZERO appointment. Then, on the basis on an in-depth medical history and an analysis of the medical record, we propose the best solution in the form of an attending physician who will be matched according to the severity of illness, age and social expectations of the patient (e.g. pregnancy, change of environment involving major life changes). The attending physicians at Neurosphera are acknowledged specialists in the field of neurology and epileptology. They constitute a team which constantly deepens its knowledge and clinical practice, as well as cooperates closely both in diagnosis and therapy adjustment. The patient can also choose their own attending physician. However, it must be noted that the Neuroterminal and its benefits put all doctors on the same level in terms of diagnosis and therapy suggestions.

The first appointment at Neurosphera is called a ZERO appointment. It can be conducted as an in-person appointment or in the form of a teleconference. The appointment in the form of a teleconference requires forwarding all of the medical records gathered so far electronically, using Neuroterminal (the system will guide you how to send the documentation).

An effective epilepsy treatment requires continuous contact between the doctor and patient and a regularity of jointly planned activities. We know that in-person appointments with specialists are not always possible. Therefore, for the comfort of our patients and a full possibility of monitoring the patient’s condition we created the Neuroterminal care system. Thanks to the registration on the patient’s panel, you are able to include all of your events in it and stay in touch with your attending doctor, who checks the patient’s condition on a regular basis and adjusts recommendations. Scheduled appointments can be conducted in person or online. Both forms of appointments have the same value.

What types of appointments are available at Neurosphera?

At Neurosphera patients can be (and already are) treated from anywhere in Poland and abroad. Any patient can choose a convenient form of appointment – an online appointment in the form of a teleconference or a traditional in-person appointment. Thanks to the Neuroterminal system the attending physician has constant contact with the patient, without the necessity of coming to Neurosphera. Modern technology means that anyone suffering from epilepsy can be a patient of Neurosphera, regardless of where they live.

The first appointment of a new patient is called the ZERO appointment. It lasts for around 60 minutes.

Subsequent appointment: the FIRST appointment lasts from 30 to 60 minutes.

REGULAR appointments last for about 30 to 45 minutes.

A CONSULTATION appointment with a doctor of another specialization lasts for about 45 to 60 minutes.

What is a CONSULTATION APPOINTMENT?

You can make two kinds of appointments at Neurosphera – in a in-person and a teleconference form.

IN-PERSON APPOINTMENT: it takes place at the clinic located in Warsaw at Wiertnicza 133. During the appointment, the doctor meets the patient directly. The physician reviews the patient’s current records on site and he/she performs any additional necessary actions (e.g. makes a diagnosis, establishes treatment). In the case of a prescription, the doctor issues the prescription on site.

TELECONFERENCE: an online appointment, during which the doctor and the patient see themselves on screen. The call takes place on the Neuroterminal and does not require an installation of any other additional software. The documentation shall be sent electronically in advance through the Neuroterminal (the patient places it on their own individual account).

Treatment at Neurosphera is covered by the Neuroterminal system. Before your first appointment at Neurosphera you have to create the patient’s profile in the system and fill in as much information as possible. You should scan and save all of the documents regarding the earlier epilepsy treatment at other centres in the right place. Thanks to that, you will not need to bring a paper version of your documents with you on your in-person appointment.

Neuroterminal

The Administrator of your personal data is Neurosphera with its registered office at ul. Wiertnicza 133, 02-952 Warsaw, registered at the District Court for the capital city of Warsaw in Warsaw, 13th Commercial Department of the National Register Courthouse under the number KRS 0000880321, REGON: 38802740300000, NIP: 9512513486.

Access to the medical record have only the doctors of Neurosphera, who use the Neuroterminal and take care or diagnose a given patient of our Centre.

You can contact your doctor through the Neuroterminal whenever you feel like doing so. When you feel the necessity of scheduling an appointment – all you need to do is choose a satisfactory form on the patient’s panel and then follow the recommendations. If you need a prescription or want to ask a quick question, just use the option “Order a prescription” or “Write a message”. However, you must remember that the question to the doctor has to be a short one, on which they can respond in a clear and brief manner. Otherwise, you have to schedule an appointment.

Doctor Zwoliński has introduced to the computer’s memory the descriptions of more than 30 medications, which are commonly used in epilepsy therapy in the country and abroad, as well as the recommendations of the world-renowned authorities and scientific societies regarding the use and dosage of medication. After entering the patient’s data and comparing them with the characteristics of all the epileptic types described by Dr Zwoliński, the doctor will make a diagnosis based on the analysis provided by the system. Then, by analysing the data concerning the medication and the worldwide recommendations in a specific disease situation and appropriate dosage, the doctor will suggest pharmacological recommendations taking into account the analysis presented by the robot. You must however remember that the final decision concerning the therapy belongs always to the doctor.

Thanks to the great computing power of the modern computers we are able to analyse millions of features in millions of cases at once, in real time. This is an undeniable superiority of the computer over the human brain. No research team is able to work on so many cases. The computer, on the other hand, is able to conduct comparative diagnosis in real time. We therefore hope that the use of computers in epilepsy therapy and diagnosis will provide opportunities for therapeutic progress in cases of some severe forms of epilepsy, in all known methods of diagnosis and therapy have failed so far. By analysing the entered data and comparing it with the software resources, doctors will notice new regularities that we were not able to notice before thanks to the support of the software. On this basis, the doctor will be able to propose new therapeutic solutions, unknown to us yet.

All of the doctors working at Neurosphera have the Ictal robot at their disposal. This brings the working models – diagnosis and proposed solutions – closer together through the standardisation of work. The knowledge included in the system has been developed on the basis of the medical experiences of doctor Zwoliński, so it can be said that any doctor can propose a lateral therapy with doctor Zwoliński’s potential recommendations, because his knowledge is passed on to other specialists on an ongoing basis. The entire therapeutic path at Neurosphera, thanks to the use of the digital tools, is strictly calculated for the highest possible comfort and convenience of the patient. If you registered in the Neuroterminal on the patient’s panel, you can see that the system is easy to use and the contact with your doctor is now simpler than ever.

It is a special module that possesses AI tools. It was developed by Piotr Zwoliński, MD, an outstanding epileptologist, Neurosphera’s medical partner and by Col. Mariusz Chmielewski, PhD, an architect of AI algorithms, technological and scientific mentor in the fields of modelling, medical diagnosis and telemedicine. Doctor Zwoliński has been cataloguing and describing various types of seizures for years. On his professional path, he has gathered dozens of them and has described each of them with a set of a hundred unique features. All of the vast gathered knowledge has been described in a mathematical way. Additionally, doctor Zwoliński has described and introduced to the system more than 30 medications, which are commonly used in epilepsy treatment at home and abroad, as well as recommendations of worldly-renowned authorities and scientific societies regarding the use and dosage of medicine. All of this gave a huge library of knowledge, which now can be used in diagnosis and treatment. After introducing the patient’s data to the computer’s memory on the medications in use, the computer will compare them with the described cases and world therapeutic recommendations in real time. Ictal supports the doctors’ work. You will not be allowed to use it by yourself.

Neuroterminal is a comprehensive care system for epilepsy patients who are treated at the Neurosphera Epilepsy Therapy Centre. Thanks to the possibilities of the system – the storage and update of the patient’s data, as well as the constant contact on the doctor-patient hotline, the treatment becomes more effective. The patient can save all of their events up to date, as well as contact the doctor at any time, ask for a prescription or update the data. The doctor has access to the medical condition in real time and can make therapeutic decisions quickly. Furthermore, besides the patient’s panel, Neuroterminal’s brain is the Ictal robot, which, thanks to its comparative abilities, is able to make the best diagnosis and precisely adjust medications.

The activity of the Neurosphera Epilepsy Therapy Centre has been established in such a way that the patient must be registered in the Neuroterminal system. It is the basis for our contact, making a suitable diagnosis and proposing and conducting therapy.

Neuroterminal is a care system, so the patient has to log in to the patient’s portal and create their own account. The system will guide and help to fill all the personal and medical data and attach the current documentation. In this way, at the very first appointment, called the ZERO appointment, the doctor will already have information about the patient, which they will supplement with an in-depth medical history. Then, the patient is able to schedule all the appointments through the platform, post all of their events and keep in touch with their doctor. The doctor will observe the medical situation of the patient. Prescriptions will be issued through the Neuroterminal as well.

At Neurosphera we provide not only the highest level of diagnosis and treatment of epilepsy, but also consultations with renowned specialists in related fields. Physicians other than neurologists and epileptologists who cooperate with Neurosphera are referred to as clinical consultants and appointments with them are called CONSULTATIONS. A neurosurgeon, neuropsychologist, clinical speech therapist and a psychiatrist cooperate with the clinic.

The appointment lasts from 45 to 60 minutes. During the appointment the patient receives medical advice in the area of specialization of the Neurosphera clinical consultant doctor.

All the appointments are arranged in a special therapeutic cycle and bear their own names. Any patient that begins treatment at Neurosphera goes through all kinds of appointments in turn:

ZERO appointment – the first appointment at Neurosphera, which lasts for about 60 minutes.
It is conducted by a clinical resident doctor. During the appointment, a thorough medical history is taken from the patient, an analysis of the medical history and comorbidities, as well as the use of all previous therapies. On the basis of the gathered data it will be possible to make a preliminary diagnosis and determine the scale of the medical problem.

FIRST appointment – an appointment during which an early diagnosis is made and an attending physician is chosen for the patient. The appointment lasts from 30 to 60 minutes. The diagnosis is supported by modern technology and an initial computer diagnosis of Ictal is also made. Furthermore, an initial therapy plan is developed during the appointment, including an examination and a follow-up plan, an Ictal Move clinical computer observation plan and an initial computer therapy plan.

REGULAR appointment – every subsequent appointment. It lasts from 30 to 45 minutes. During the appointment, a final diagnosis is established and then the data gathered in the Ictal system are analysed. At regular appointments, the doctor decides on the next therapeutic steps. It is possible to intervene if there is no improvement or if the condition gets worse through an Ictal consultation or a joint consultation.

All of the patients treated at Neurosphera will be covered by the Neuroterminal system. Thanks to this, they will keep in touch with the doctor and the regularity of the appointments will be maintained. Neuroterminal provides continuity of contact and a full possibility of monitoring the patient’s condition. That way, patients from all over the country, as well as from abroad, can be treated at our clinic.

The first step for the patient is to register through the Neuroterminal on the patient’s panel. Thanks to the possibility of posting personal and medical information as well as medical records, it will be easier to make a diagnosis and propose treatment. During the therapy, on the panel you can post all your events and stay in touch with the attending physician, who constantly checks the condition of the patient and adjusts the recommendations. Appointments can be conducted in person or online. Both forms of appointments have the same value.

The patient via Neurosphery can make an appointment for the “zero” visit (new patients) or a migration visit (existing Neurosphera patients), and then for the first visit and for subsequent visits

How to do it?

  1. On the main page of your patient panel, click on Make an appointment.

  2. Select the doctor and the type of visit: teleconference or stationary visit.

  3. Then set the day and time of your appointment using the calendar of available appointments with the selected doctor.

  4. A summary of information about your planned visit will appear. Check and confirm your choice by clicking “Pay”.

  5. The system will take you to the payment selection page. Select a payment method and follow the instructions to pay for your visit.

  6. Once the appointment has been paid, a confirmation message will appear on your patient panel with the details of the appointment. You will receive an SMS reminder 24 hours before your appointment.

  7. A scheduled appointment is visible on your panel in the “Upcoming appointments” tab. 8.

  8. You may change the date of your planned visit: click the edit icon, select a new time of your visit and confirm.

  9. You also have the option of cancelling your appointment. Just click the bin icon and confirm the cancellation of your appointment.

Important information. A patient cannot make more than one appointment at a time.

If you have not been a Neurosphera patient so far or you want to add the results of new tests or, for example, a recording of the last episode made with your mobile phone, you will do it using the “Medical records” tab.

  1. Log in to your patient panel and on the home page select “Edit” under the Medical Records section.

  2. Go to the Add document section and then

    • Select the type of document you wish to add: e.g. medical history, video event or imaging study description.

    • Enter a name for the document you are adding (e.g. seizure video of 12.05.21)

    • Add the date of the event (examination, result, seizure)

    • Attach the file using “drag and drop” or “select a file from your computer” and click “attach”. The file will be attached to your records.

    • Add all other documentation items in the same way.

    • If you do not know how to assign a document, select “Other”.

    • You can edit any of the attached documents at any time, you can also view the contents of your “folder” of documents or delete the attached file.

  1. Important! Even if you have written down a record, document or even the dosage of a medicine on a piece of paper, photograph it and attach it as well. Every piece of information is valuable to your doctor. The more complete the documentation, the better the quality of your visit.

Filling in your personal data is your ticket to all the functions of your patient panel. Only after entering your details can you attach documentation, make an appointment or contact your doctor.

Here we go!

  1. Log in to your patient panel using the login and password you defined when you registered.

  2. Click on the “Edit” button under the “My Details” section.

  3. On the data page, click the green “Edit” button again and proceed to complete your personal information.

  4. All fields marked with an asterisk are mandatory and must be completed as indicated.

  5. It is very important to mark in the data field if the patient was previously treated in Neurosphera. Then the so-called migration visit will be prepared, during which all the previous documentation will be transferred by the doctor to the new system.

  6. Selecting “Yes”, “No” or “I don’t know” in the question about suffering from epilepsy will allow us to direct you to the appropriate specialist and select the appropriate stage of consultation.

  7. If you are registering a minor or incapacitated patient, please also complete the section entitled “Information on legal representative”.

  8. Be sure to indicate which form of legal guardianship you have.

  9. If you wish to enter your invoice details, please tick the visible box and complete the invoice details form below.

  10. Information about your blood group is optional, but useful.

  11. Be sure to fill in your height and weight.

  12. Finally, fill in your contact details: name, surname and telephone. This information is not compulsory, but is very useful in an emergency.

  13. When you have completed all the information, click on the “Submit” button.

  14. You now have access to the entire patient menu: you can make an appointment, upload your records, write a message to your doctor, etc.

  15. You can edit or complete your personal details at any time by simply clicking on “Edit”.

Before you start using the Neuroterminal patient panel you need to register or create an account. The registration is free and you can do it in an easy way.

Only when you have a registered account you can add your personal data, attach the documentation, make an appointment or contact the doctor.

Here we go!

  1. Click on the REGISTER SINGLE button in the top menu

  2. Enter your name, surname, email address, which will also be your login, and define a password to log in to your patient panel. The password should contain at least 8 characters, one special character, one capital letter and one number.

  3. Accept the terms of service, read the rules of personal data processing and tick the box “I am not a robot”. Tick the marketing consents if you want us to keep you informed about our activities.

  4. Click on the REGISTER button.

  5. You will receive an email confirmation of your booking to the email address you provided in your registration details.

  6. Now go to your e-mail box, open the e-mail from the Neuroterminal registration system and click on the confirmation link. You will be taken to the Neuroterminal website, with confirmation of the completion of the registration process. Note, sometimes the registration confirmation email may end up in spam, so check your inbox carefully.

  7. If you want to use your patient panel right away, log in using the data you provided during the registration (login + password)

  8. After logging in you will see the main page of your patient panel (“About me”).

  9. In order to use all the functions of your panel, you must fill in your personal data. See how to do it, click “Complete personal data”.

About the clinic

Neurosphera is accessible to people with disabilities. The entrance to the building where Neurosphera is located is accessible from the level of the pavement. There is a lift inside which fits a patient in a wheelchair. On the floors where Neurosphera’s offices are located there are no obstacles for people with disabilities.

There are marked parking spaces available for patients on the property where Neurosphera is located, to the left of the gate.

For those using navigation: we ask you to type the Wiertnicza 133 address. For those using the Google Maps app on smartphones: all you need to do is enter “Neurosphera”.

It will take 30 to 40 minutes by public transport from the centre of Warsaw or the Central Railway Station.

You can easily get to Neurosphera both by car and by using public transport. The clinic is located at a big street, directly linked to the A2 highway and roads leading out of town and in the extension of the water route along the Vistula river, thanks to which the patients arriving at Neurosphera from northern directions will easily and comfortably get to the place.

You can get tot Neurosphera from the Central Railway Station directly by taking the bus line 131 (destination Sadyba), which will get you to the terminal Sadyba and then walk along Wiertnicza street in the direction of Wilanów to the house number 133 or from the city centre (stop Rondo de Gaulle’a) by taking the bus line 180 in the direction of Wilanów to the Łowcza stop. After leaving the bus, you need to go back a few numbers.
